In order to participate in Pre-Season Conditioning:
Players must be registered prior to conditioning -- scroll back up to the large orange button and register.
Players must be members of US Lacrosse. Membership expiration dates must run through 6/3/2011. To become a member of US Lacrosse or to renew your membership please visit http://uslacrosse.org/ Please print confirmation page once membership is confirmed/updated.

Assumption of Risk Form- if you have EVER played a sport at Barrington Middle School, you should have already filled this out. If you have not played a sport at Barrington Middle School, please complete TWO of these forms and have BOTH notarized (Town Hall will notarize -- Assessor's Office on 2nd Floor). Parent Permission Form - this must be completed by EVERYONE who is playing lacrosse this year. Both parents and students must sign the form. Parent Permission and Acknowledgement Form.pdf
Student Athlete Handbook - In order to complete the Parent Permission form, both the parents and students must read the Barrington High School Student Athlete Handbook. 2010-2011 Athletics Handbook.pdf

Pick Up Authorization Form-must be completed if anyone other than parent of player is providing transportation. BMS_Girls_Lacrosse_2011_Pickup_Authorization_Form.pdf
Physical Form - Must be signed by your doctor and dated No Earlier Than 6/3/2010. If you have already played a sport at Barrington Middle School this school year (September 2010-present) you turned in this form but it is your responsibility to be sure the date on the form is accurate. Concussion Pamphlet and Concussion Awareness Statement (NEW FOR THE 2010-2011 SCHOOL YEAR) - In compliance with the recently passed legislation (School & Youth Programs Concussion Act Title 16-90-1) all Rhode Island schools/student athletes must adhere to the following requirements:
1. Parents and Athletes must review and sign the attached Pamphlet: Concussion Pamphlet.pdf (for additional information please visit www.sportsconcussions.org)
2. Parents and Athletes must review and sign the Concussion Awareness Statement Concussion Awareness Statement.pdf
School Insurance: Every student athlete must be covered by health insurance, either through their family or through the school. All students received the school insurance forms at the beginning of the school year and were required to return them to school whether or not they wanted to purchase insurance. If you did not complete this form, Insurance pamphlets are in the Main Office at the HIGH SCHOOL and in Mr. Finn's office (the Athletic Director).

UNDER NO CIRCUMSTANCES WILL STUDENTS BE PERMITTED TO PARTICIPATE IN PRE-SEASON CONDITIONING UNTIL WE HAVE VERIFIED THAT THEIR FORMS ARE COMPLETED.
Pre-season Conditioning
March 7 and 9th 2:30-4:00pm ONLY*
*Please note this is a change from the original communication due to the availability of the gym.
Pre-season conditioning sessions are indoors and MANDATORY TO PARTICIPATE IN THE 2011 SEASON-sneakers(NO CLEATS), sticks, goggles, mouth guards, NO CAMIS, and NO ROLLING SHORTS
Tryouts: March 14th and 16th(Monday and Wednesday) 3:30-5:00pm

Players must register for their team online and pay on or before Friday, March 25, 2011.
