Welcome to our online sign up. Here you can enter your information and then print out the forms needed to bring to us with your payment and a copy of the players birth certificate. With you filling this out online this helps us with proper spelling of names and helps us with correct phone numbers and email addresses. Click the link on the left to go back to our home page. Once you print out the forms please sign and initial were needed and return with your payment. Also please note print all forms and there are items needed to be filled out by your doctor. Payments can be made in cash, check or charge by using the PayPal link on our web page.
