These teams will participate in the East Hudson Youth Soccer League.

All new players to travel soccer should submit to the coach or team manager

1) a copy of birth certificate to show proof of age

2) a passport sized photo ( headshot type) name and team on back

3) a signed medical release form- which can be found on the Lewisborosoccerclub.org website

Any players who have played on a travel soccer team in the past, with EHSYL, will need to submit

1) a passport sized photo ( headshot type) name and team on back

2) a signed medical release form Any player who did not pay the registration fee online after registering, should send a check payable to LSC for $125. Please be sure to indicate FC LEWISBORO EXTREME on the check.

These fees are used to cover the EHYSL and admin cost associated with these teams. All other expenses related to the teams in this club are the responsibility of the player- these include, but may not be limited to, professional trainer fees, tournament costs, uniform and warm- up costs, travel expenses.
