All paperwork needs to be emailed to MFYFL.  Email address is registrations@mfyfl.com.  If paperwork is not received by June 30th, registration will be cancelled and payment refunded.*****

FORMS DUE ARE BELOW.  PRINT NOW OR GO TO www.mfyfl.com forms section to download and print:

• Copy of birth certificate if new to MFYFL
• Registration packet
• Volunteer Form (if volunteering) Release of Liability for adult participants (if volunteering)

Before season begins, please have your child complete their physical exam.  Download and print the physical exam form and take it to your child's doctor.
