MAYF's 2012 Football and Cheering season!
Early and guaranteed registration is available Feb. 6th through May 19th.
After May 18th late registration is available for $150.
There is a $100 discount for each registrant after the second registration.
To complete registration the following must be received, participants will not receive their equipment or uniforms until registration is complete!:
- Arrange for payment of the registration fees before the first practice.
- Your equipment from 2011 MUST be turned in to complete your 2012 registration.
- The Summary of Releases form.
- An original birth certificate with a raised seal (if one is not on file already).
- Your ORIGINAL year end report card from school (June, 2012).
- A Doctor's Release Form dated on or after January 1st, 2012.
- Additional forms are required for coaching/volunteer applications. Coaching applications received after June 1st may not be processed in time for the season.
Note: We will be conducting a "live" 2012 registration and 2011 equipment return on March 24th from 10am-2pm, at Ward Park's Blockhouse.
