2012 FALL BALL
$45 for T-Ball/Coach Pitch and $75 for Majors/Minors
Registration Fee includes uniforms and 8 weeks of instruction by Coach Eric Kibler and the Horizon High School Coaching Staff.
Online Registration Information
· All families will be required to create an account the first time logging into our system. Please add all family members, players and parents by choosing the add children or add adult options (alternate contact information such as phone numbers and emails can be added when another adult is added to the account).
· If you have multiple players, please register them all at the same time otherwise discounts will not be applied. To do this after completing one registration click the continue shopping button to register another player
· After the account is created the registration system will be used to register and make payments for any of the available programs. Through the registration system you will have access to all account information, including payments due, payments received, due dates, outstanding balances, etc.
· PVNLL accepts ALL Major Credit Cards as well as Checks Online.
· All players must provide a copy of the birth certificate and proof of residence within the PVNLL boundary. Proof of residency must include valid picture ID of parent or guardian with current address, plus two different items of the following - utility bill, insurance statement, voter registration, tax statement, etc.
· Adults wishing to volunteer must register for the 2012 Spring Season. There is NO fee for registering to Volunteer.
· The entire registration process occurs over a secure server using the highest security currently available.
Boundaries
Before you register, please make sure you live within the PVNLL Boundaries. All player must reside within these boundaries.
Division Selection
Although it is within the discretion of PVNLL to place children in the proper division, we still ask that you check the category/division that you feel best fits your child's age and skill level. Final placement will be made following the Skills Evaluation in January.
Financial Aid
It is the policy of PVNLL not to exclude participation because of financial reasons. If you should need assistance, please call Rich Solar at 602-363-0070.
Volunteers
If you are considering volunteering as a coach, manager, concession helper, or anything that puts you in direct contact with the children, you must fill out a Volunteer Application and submit it to PVNLL along with a copy of your driver's license. Please submit these forms during open registration or the Skills Evaluation. You will not be permitted to help on the field unless we have these documents on file.
Medical Release Form
Please complete, print and sign a Medical Release Form for each child registered. This form is to be turned into your Team Manager at the first practice.
