Getting Help from Westborough Youth Football and Cheer

Welcome to the 2011 Westborough Youth Football and Cheer Registration!
As a member of AYF and Central Mass Youth Football and Cheer Conference (CMYFCC), Westborough Youth Football and Cheer Organization provides an opportunity for all children regardless of gender, race, ethnicity, or religious preference an equal opportunity for participation in a safe, competitively balanced and organized setting.
To complete registration the following must be received:
1. Participant, Tracking and ID Card
2. Medical Clearance form
3. Emergency Medical Treatment, Consent and Information
4. Waiver and Release of Liability
5. Image Release
6. AYF Code of Conduct
(You will find the above forms as attachments when you register.)
7. An original birth certificate with a raised seal (if one is not on file already)
8. Year end report card (June 2012)
9. A doctor's note or physical dated on or after January 1st, 2012
