• Worcester Cowboy AYF Registration Form

• Registration Contract &Certification Sign-Up Form
• Medical Clearance Form ( Must be signed after January 1st of current year)
• Summary of Releases
• Waiver and Release of Liability- Minor
• Image Release- Minor
• Emergency Medical Treatment, Consent and Information
• AYF Code of Conduct Signature Page (Parents keep copy of Code of Conduct, Zero Tolerance Policy)
• Report Card- 6/2011 Copy accepted –front to back, Grade 2 - 7, 8th grade must have original and all players/cheerleaders must have a 70% grade averarage or above otherwise you will have to have a scholastic eligibility form signed by the principle).
• Original Birth Certificate W/Seal ( If return player we already have it.)
